REGISTRATION OVERVIEW
Save $100/child — IF YOU REGISTER BY MAY 6, 2012
ALL forms and applicable tuition fees must be received by May 1. This enables the school to plan for teachers, programming and space needs. We strongly encourage families to take advantage of the early bird rate to help us provide an optimal learning experience for your child. No exceptions for current families– if the school does not receive the completed forms and payments by May 6, 5:00 pm no discount will be applied.
NEW! DJCS Referral Incentive Program
$100 off enrolment fees for one child in your family per each new student registered as a result of your referral! Ensure they write who referred them on Form 1 of the Registration Form so that you are eligible for your incentive.*
• Current DJCS family that refer a new registered student will receive $100 off enrolment fees.
• If fees are already paid, a credit will be applied to next year’s tuition. The referral incentive program does not impact the $100/child EARLY BIRD discount for registration completed by May 6, 2012.
Instructions for Submitting Registration Forms and Payment
Submit forms 1, 2 and 3 with applicable fees, payable to DJCS. Mail to:

- We would appreciate full payment for each applicant at time of registration. However, if payment in installments is required, the school will accept up to three cheques for the full amount. The first cheque must be dated May 6, 2012 and the full registration amount paid by July 1, 2012.
- An administrative fee of $5 per post-dated cheque will be charged and must be paid when your registration forms are submitted.
- Cheques returned as NSF are subject to a $40 fee per cheque.
